The total cost of a California medical card has two parts: the doctor's evaluation and the optional state MMIC. Here's what each runs โ and how quickly the tax savings pay it back.
The two costs
1. The physician evaluation
An online telehealth evaluation with a licensed California provider is a modest one-time fee. This gets you the medical recommendation that lets you shop as a patient. Renewals are typically required annually.
2. The state MMIC (optional but recommended)
To also skip the state excise tax at checkout, you'll want the state-issued Medical Marijuana Identification Card (MMIC), which you apply for through your county. County fees vary (often in the range of tens of dollars, with reduced fees for Medi-Cal patients).
Do you need both?
A physician's recommendation alone lets you purchase as a medical patient. But the tax exemption specifically requires the state MMIC. If your goal is the ~15% savings, budget for both.
How fast does it pay back?
Here's the part that matters: for a regular buyer spending, say, $300/month, the excise-tax exemption alone is roughly $540/year. That typically recoups the evaluation and MMIC fees within the first month or two โ everything after that is money kept.
Ongoing costs
Plan for an annual renewal of both the recommendation and (if you keep it) the MMIC. Even with renewals, frequent buyers usually come out well ahead.
